Regional Care Coordinator

Job Summary

Managing the member experience through transportation coordination, the full-time Regional Care Coordinator will monitor trips, provide member support, and ensure successful outcomes while working remotely from Florida or Mississippi.

Key responsibilities

• Act as a liaison with health plan clients, members, and partners to coordinate care for members needing extra support
• Monitor and confirm trips, handle escalations, and ensure accurate transportation arrangements
• Document issues and follow up with members to ensure timely and safe transportation to appointments

Required qualifications

• High school diploma or G.E.D. equivalent
• 9 months of experience in a Customer Care Representative role or equivalent experience in health care case management
• Proficient in Microsoft Outlook, Word, and Excel
• Ability to handle escalated situations with analytical thought processes
• Strong leadership and team-building skills
