Job Summary
To support comprehensive diabetes and cardiometabolic care, the full-time Registered Dietitian Diabetes Specialist will deliver quality diabetes self-management education and medical nutrition therapy, utilizing both in-person and telehealth methods while collaborating with healthcare teams to optimize patient outcomes in a remote setting.
Key responsibilities
- Provides diabetes self-management education and medical nutrition therapy based on individual assessments in both group and individual settings
- Develops personalized care plans and promotes successful self-management through comprehensive patient assessments
- Collaborates with healthcare providers to create and implement patient-centered diabetes plans and participates in quality improvement initiatives
Required qualifications
- Bachelor's degree in Nutrition/Dietetics
- 1-2 years of experience as a registered dietitian
- Valid Registered Dietitian license
- Certified Diabetes Care and Education Specialist (CDCES) preferred or must obtain within 24 months of hire
- Experience in diabetes self-management education and support preferred
