Job Summary
Managing a portfolio of clients remotely, the full-time Registered Nurse Advisor will educate clients, advocate on their behalf, and coordinate care to help them navigate the healthcare system effectively.
Key responsibilities
- Build trusted relationships with clients and guide them throughout their healthcare journey
- Coordinate care by helping clients access appropriate providers and services while ensuring smooth transitions
- Support the Triage Team by welcoming and educating new clients about PHM's services
Required qualifications
- Active Registered Nurse license in the state of residence with at least five years of healthcare experience
- Experience in case management, care coordination, or a related clinical environment
- Strong clinical judgment and critical thinking skills
- Proven ability to work collaboratively and manage time effectively in a remote setting
- Experience with technology and systems relevant to healthcare delivery
