Registered Nurse Care Advocate

Job Summary

Offering a flexible 1099 contract opportunity, the Registered Nurse Care Advocate will manage member assessments, care coordination, and education for older adults and Medicare beneficiaries, all conducted remotely via phone or video.

Key responsibilities

• Conduct phone and video assessments to develop and update member-centered care plans
• Coordinate with healthcare providers and community resources to address care gaps and support transitions
• Educate members on their care plans and advocate for their needs while documenting interactions accurately

Required qualifications

• Active, unrestricted RN license in at least one U.S. state; multistate license preferred
• 3+ years of RN experience in care management, case management, or related fields
• Demonstrated ability to build trust with older adults, including those who may be skeptical or confused
• Proficiency with EHRs and communication tools, with the ability to learn new systems quickly
• Self-directed with a reliable computer, high-speed internet, and a private workspace
