Registered Nurse Care Manager
Job Summary
To support a growing healthcare initiative, the full-time temporary Registered Nurse Care Manager will coordinate care for members with complex chronic conditions, conduct assessments, and develop individualized care plans while working remotely from Monday to Friday, 8:00 am to 5:00 pm EST.
Key responsibilities
- Conduct comprehensive assessments and develop individualized care plans for members
- Engage in telephonic member coordination, dedicating 50-75% of the workday to this task
- Document care management activities accurately in the electronic health record and collaborate with interdisciplinary care teams
Required qualifications
- Active, unrestricted Compact Registered Nurse (RN) license in the candidate's state of residence
- Associate's Degree in Nursing; Bachelor's Degree preferred
- 3+ years of nursing experience and 2+ years in case management or related fields
- Proficiency in Microsoft Office Suite (Word, Excel, Outlook, OneNote, Teams)
- Experience with Medicare and/or Medicaid care management is a plus
