Registered Nurse Care Manager

Job Summary

To support chronic disease management initiatives, the full-time Registered Nurse Care Manager will conduct comprehensive telephonic assessments, collaborate with interdisciplinary teams, and manage patient referrals in a remote work environment.

Key responsibilities

• Provide chronic disease management support for value-based care contracts, including patient education and care planning
• Conduct comprehensive telephonic assessments to identify medical, psychosocial, behavioral, and environmental needs affecting patient health outcomes
• Collaborate with an interdisciplinary care team to coordinate patient-centered care and manage registry-based referrals

Required qualifications

• Active Registered Nurse license obtained prior to hire date
• Diploma from an accredited school/college of nursing
• 3-5 years of experience in care management, case management, social services, community health, or clinical practice
• Proficiency in electronic health records and care management software
• Certification in Case Management (CCM, ACM, or similar) preferred
