Job Summary
To support a growing healthcare team, the full-time Nurse Care Manager specializing in Medicare and Medicaid will coordinate care for members with chronic conditions, conduct assessments, and develop individualized care plans while working remotely.
Key responsibilities
- Conduct comprehensive assessments to identify member needs and connect them with healthcare benefits and community resources
- Develop individualized care plans based on clinical information and collaborate with interdisciplinary teams to address barriers to care
- Document care management activities accurately in electronic health records and ensure compliance with regulatory requirements
Required qualifications
- Associate's Degree in Nursing; Bachelor's Degree preferred
- Active and unrestricted Compact Registered Nurse (RN) license in the state of residence
- 3+ years of nursing experience
- 2+ years of case management or home healthcare coordination experience
- Proficiency in Microsoft Office Suite, including Word, Excel, Outlook, OneNote, and Teams
