Job Summary
Managing medically complex patients, the full-time Registered Nurse Care Manager - Compact License will provide telehealth care management, prepare Annual Wellness Visit charts, and support preventive care initiatives in a fully remote setting with evening and overnight shifts.
Key responsibilities
- Prepare comprehensive Annual Wellness Visit charts and communicate findings to providers
- Conduct nursing assessments and develop individualized care plans for chronic care management
- Identify and close care gaps while coordinating care across multiple services and providers
Required qualifications
- Active, unrestricted Registered Nurse (RN) license in a compact state
- Minimum 3 years of clinical nursing experience, including telehealth or virtual care settings
- Experience in Chronic Care Management (CCM) or Remote Patient Monitoring (RPM)
- Proficiency with electronic health record systems and telehealth platforms
- Ability to work independently in a remote environment with a dedicated workspace
