Job Summary
To support future patient care initiatives, the full-time Remote Registered Nurse Care Manager (OR, WA, AK) will manage patients with complex chronic conditions, collaborating with healthcare providers to develop and implement comprehensive care plans aimed at optimizing patient outcomes.
Key responsibilities
- Quickly build empathetic relationships with patients and families to assess their needs based on individual values and preferences
- Interface with healthcare providers to develop and implement effective case management plans
- Monitor care effectiveness and coordinate patient admissions and discharges across various healthcare settings
Required qualifications
- Active Registered Nurse license in Washington, Oregon, and/or Alaska
- Experience engaging patients virtually using various technologies
- Bachelor's of Science in Nursing preferred
- 3+ years of clinical practice in a healthcare setting highly preferred
- Prior experience with electronic health records or health registries required
