Registered Nurse Case Manager

Job Summary

To support beneficiaries in regaining optimum health, the full-time Remote Registered Nurse Case Manager will perform comprehensive assessments, develop care plans, and coordinate care while advocating for self-care management and access to healthcare resources.

Key responsibilities

• Conduct comprehensive assessments to identify clinical, psychosocial, and behavioral concerns and develop individualized care plans
• Coordinate and implement care plan activities, ensuring continuity of care and effective communication among the healthcare team
• Monitor and evaluate care plans regularly, revising them as necessary to achieve desired health outcomes

Required qualifications

• Current, unrestricted Registered Nurse license in the state of residence with the ability to obtain multi-state licensure
• Associate or bachelor's degree in nursing from an accredited institution; bachelor's degree preferred
• 5+ years of clinical RN experience in direct patient care
• Proficiency in Microsoft Office and mobile technologies
• Ability to obtain Security Clearance; current DOD Security Clearance preferred
