Registered Nurse Case Manager

Job Summary

Coordinating the interdisciplinary plan of care for patients, the full-time Registered Nurse Case Manager will monitor patient care from admission to discharge, acting as a liaison among patients, families, and healthcare personnel while working remotely.

Key responsibilities

• Identifies patient risk factors and obstacles to care, discharge, and readmission
• Evaluates and collaborates on the plan of care with the medical team, ensuring timely patient movement through the healthcare system
• Educates patients and families about the progression of care and facilitates ongoing insurance authorization for continued stay

Required qualifications

• Bachelor's Degree in Nursing
• Current Registered Nurse (RN) license
• Minimum of 2 years of relevant experience in nursing or case management
• Knowledge of insurance reimbursement policies
• Experience in coordinating patient care and discharge planning
