Registered Nurse Case Manager

Job Summary

To support value-based care initiatives, the full-time Remote Registered Nurse Case Manager will manage chronic disease support, conduct comprehensive patient assessments, and collaborate with an interdisciplinary care team to coordinate patient-centered care.

Key responsibilities

• Provide chronic disease management support and patient education for value-based care contracts
• Conduct telephonic assessments to identify patients' medical, psychosocial, and environmental needs
• Collaborate with an interdisciplinary team to ensure coordinated and comprehensive patient care

Required qualifications

• Active Registered Nurse license obtained prior to hire
• Diploma from an accredited school/college of nursing
• 3-5 years of experience in care management, case management, or clinical practice
• Proficiency in electronic health records and care management software
• Certification in Case Management (CCM, ACM, or similar) preferred
