Registered Nurse Clinical Referral Coordinator

Job Summary

To support the Home Health and Hospice service lines, the full-time Clinical Referral Coordinator Nurse will manage clinically complex referrals, coordinate communications with referral sources, and ensure compliance with regulatory requirements in a remote work environment.

Key responsibilities:

• Acts as the clinical liaison for referrals, promoting continuity of patient care with physicians, hospitals, and insurance case managers
• Responds to inquiries regarding referrals and ensures accurate documentation for the admissions process
• Participates in policy development and problem-solving to enhance the clinical intake function

Required qualifications:

• Completion of an accredited school of professional nursing
• 1 year of experience in home health or hospice care strongly preferred
• Current Registered Nurse (RN) licensure
• Clinical knowledge and critical thinking ability to coordinate patient care effectively
• Clearances including Act 34 and OAPSA
