Job Summary
Working remotely in a full-time capacity, the Registered Nurse Telephonic Care Manager will manage patient transitions of care following significant healthcare events, conducting comprehensive assessments, developing individualized care plans, and coordinating resources to ensure optimal patient outcomes.
Key responsibilities
- Conduct comprehensive assessments to identify patient needs and develop individualized care plans
- Coordinate transitions of care across various healthcare settings and arrange necessary follow-up services

Required qualifications
- Active Registered Nurse (RN) licensure
- Bachelor's Degree in Nursing preferred
- Five (5) years of experience in care management or related clinical experience
