Remote Bilingual Care Coordinator
Job Summary
To support its Special Needs Program, the full-time Remote Bilingual Care Coordinator (Spanish or Vietnamese) will manage outreach to members, assist with care transitions, and collaborate with RN Case Managers to address complex health needs in a remote environment.
Key Responsibilities
- Conduct outreach to members for referrals, authorizations, and care needs
- Complete assessments in the Case Management system and monitor alerts for changes in member conditions
- Act as a liaison and advocate for members, ensuring effective communication with providers
Required Qualifications
- Bilingual in Spanish or Vietnamese (required)
- Minimum 1 year of experience in a healthcare IPA setting
- Minimum 1 year of experience with referrals and prior authorizations
- High school diploma or GED (Bachelor's degree preferred or 4 additional years of experience in lieu of degree)
- Basic computer skills (Microsoft Outlook, Word, Excel); typing speed of 25+ WPM
