Remote Clinical Pharmacist

Job Summary

To support a variety of clinical programs, the contract Remote Clinical Pharmacist will engage members through telephonic consultations, conduct comprehensive medication reviews, and provide medication adherence support while working remotely on a flexible schedule.

Key responsibilities

• Engage members telephonically and schedule pharmacist consultation appointments for clinical program participation
• Conduct comprehensive Medication Therapy Reviews and provide clinical recommendations for medication management
• Educate members on their medications, regimens, and related disease states to enhance adherence and health outcomes

Required qualifications

• Current and unrestricted pharmacist license in the state of residence
• Strong knowledge of disease-state management guidelines and Medication Therapy Management (MTM) principles
• Approximately 1+ year of pharmacy experience preferred; new graduates may be considered
• Ability to navigate multiple databases and technology applications effectively
• Intermediate proficiency with Microsoft Word, Outlook, and Excel
