Remote LCSW Mental Health Case Manager

Job Summary

Providing compassionate care remotely, the full-time Remote LCSW Mental Health Case Manager will conduct assessments, develop treatment plans, and coordinate care for patients and families facing behavioral health challenges.

Key responsibilities

• Conduct behavioral health assessments and create individualized treatment plans
• Provide individual, family, and crisis intervention services while advocating for patient participation in treatment goals
• Coordinate care with interdisciplinary teams and maintain accurate clinical documentation in Epic

Required qualifications

• Master's degree in Social Work, Counseling, or a related human services field
• Licensed Clinical Social Worker (LCSW) certification
• 2-3 years of behavioral health experience preferred
• Strong skills in assessment, crisis intervention, and care coordination
• Proficiency with Microsoft Office and electronic medical records
