Remote Oncology Case Manager
Job Summary
A company is looking for a Remote Oncology Case Manager (Transitions of Care).
Key Responsibilities
- Conduct clinical assessments and monitor changes in health for members diagnosed with cancer
- Coordinate care transitions and educate members and caregivers about their diagnosis and treatment
- Assist with administrative projects and communicate feedback to improve service offerings
Required Qualifications
- Bachelor of Science Degree in Nursing and an unrestricted Registered Nurse (RN) license
- At least 5 years of nursing experience, including 3 years in oncology nursing or case management
- Certification as a Certified Case Manager (CCM)
- Ability to manage multiple tasks and work under pressure while maintaining organization
- Comfort with new technologies and electronic health records
