Job Summary
Seeking a Rhode Island Licensed Clinical Pharmacist, the full-time position will manage and optimize anticoagulation therapies for patients, ensuring safe medication use while collaborating with healthcare teams in a hybrid on-site/remote work environment.
Key responsibilities:
- Apply pharmacotherapeutic principles to ensure safe and effective medication use for patient populations
- Review and approve patient medication orders, ensuring therapeutic appropriateness and optimizing drug regimens
- Provide education on medication use to patients and healthcare practitioners, and participate in quality improvement initiatives
Required qualifications:
- Bachelor of Science in Pharmacy or Doctor of Pharmacy degree from an accredited college of pharmacy
- Current unrestricted licensure as a pharmacist in the State of Rhode Island
- Two to three years of related experience in a complex healthcare environment
- Knowledge of pharmacy information systems and electronic health records
- ACPE-approved Immunization Certification and American Heart Association Basic Life Support Certification within six months of start date for applicable positions
