Job Summary

To support a progressive pharmacy-managed medication reconciliation practice, the full-time Rhode Island Licensed Medication Technician will remotely obtain and compile detailed home medication histories and allergy information for patients admitted to the hospital, ensuring accurate documentation in the electronic medical record (EMR).

Key responsibilities:
  • Accurately obtain and compile current home medication histories and allergy information through patient and family interviews, and communication with outpatient pharmacies and providers
  • Document detailed medication lists and allergy information in the EMR, ensuring the highest level of accuracy and timely follow-up on discrepancies
  • Participate in quality assurance and improvement activities related to medication safety and reconciliation practices
Required qualifications:
  • High school graduate or equivalent; completion of one to two years of college preferred
  • Current unrestricted Rhode Island Pharmacy Technician licensure and National Pharmacy Technician Certification (CPhT) or equivalent experience
  • Minimum one-year pharmacy technician experience in an outpatient pharmacy setting
  • Strong customer service skills with the ability to interact effectively with patients, families, and healthcare providers
  • Proficient in computer skills, including Microsoft Office Suite and electronic medical record systems
