Job Summary
To support a growing telehealth initiative, the full-time Rhode Island Licensed Telehealth Physician will collaborate with healthcare advocates to facilitate patient care, oversee care plans, and address social determinants of health in a remote setting.
Key responsibilities
- Learn and utilize Solace systems and tools while providing clinical expertise in patient interactions
- Identify and prioritize patients' needs to maintain a streamlined care continuum
- Oversee patient care plans in collaboration with healthcare advocates to address various social determinants of health
Required qualifications
- Board certification in family medicine, internal medicine, or a relevant specialty
- DEA license and HAP certification
- Strong clinical skills with excellent organizational abilities
- Experience working with diverse patient populations, including elderly patients
- Technical proficiency and adaptability to new systems and software
