RN Care Navigator

Job Summary

To support kidney health management, the remote RN Care Navigator will manage a caseload of members, facilitating high-quality care through case management activities, patient education, and collaboration with healthcare teams.

Key responsibilities

• Handle in and outbound calls to provide exceptional service and education regarding kidney health and treatment options
• Engage members in HealthMap's Kidney Health Program and follow up based on policy guidelines
• Coordinate care among patients, their support networks, and healthcare providers to address individual needs and goals

Required qualifications

• Active, unrestricted RN license required
• Bachelor's degree required; 6+ years of RN experience including 3+ years in case management may be considered in lieu of degree
• CCM certification preferred
• Three (3) years of experience in case management preferred
• Experience in a dialysis center or transplant center preferred
