RN Care Navigator

Job Summary

To support kidney health management, the full-time RN Care Navigator will manage a caseload of patients, facilitating care coordination and promoting positive outcomes while working remotely.

Key responsibilities

• Handle in and outbound calls to provide exceptional service and educate members on kidney health and treatment options
• Engage members in HealthMap's Kidney Health Program and follow up based on complexity and policy guidelines
• Collaborate with interdisciplinary care teams to coordinate care and resolve patient concerns, while complying with HIPAA and operational policies

Required qualifications

• Active, unrestricted RN license required
• Bachelor's degree required; 6+ years of RN experience including 3+ years in case management may be considered in lieu of degree
• CCM certification preferred
• Three (3) years of experience in case management preferred
• Experience in a dialysis or transplant center preferred
