RN Care Navigator

Job Summary

To support kidney health management, the fully remote RN Care Navigator will manage a caseload of patients, facilitating care coordination, conducting health screenings, and collaborating with healthcare providers to ensure quality outcomes.

Key responsibilities

• Handle inbound and outbound calls to provide exceptional service to members
• Educate patients on kidney health, dialysis, and transplant options while engaging them in HealthMap's Kidney Health Program
• Coordinate care by acting as a liaison between patients, their support networks, and healthcare providers

Required qualifications

• Active, unrestricted RN license
• Bachelor's degree or 6+ years of RN experience including 3+ years in case management
• CCM certification preferred
• Three years of experience in case management preferred
• Experience with nephrology or cardiology patients preferred
