RN Case Manager

Job Summary

To support kidney health management, the remote RN Case Manager will manage a caseload of members, conduct health screenings, create care plans, and collaborate with healthcare providers while working within Pacific and Mountain time zones.

Key responsibilities

• Handle inbound and outbound calls to provide exceptional service to members
• Educate members on kidney health, dialysis options, and engage them in HealthMap's Kidney Health Program
• Coordinate care as a liaison between patients, their support networks, and healthcare providers

Required qualifications

• Active, unrestricted RN license required
• Bachelor's degree required; 6+ years of RN experience including 3+ years in case management may be considered in lieu of degree
• CCM certification preferred
• Three (3) years of experience in case management preferred
• Experience in a dialysis center or transplant center preferred
