RN Case Manager

Job Summary

To facilitate high-quality kidney health management, the fully remote RN Case Manager will manage a caseload of mixed acuity members, conduct health screenings, create care plans, and collaborate with healthcare professionals to ensure effective patient care.

Key responsibilities

• Handle inbound and outbound calls to provide exceptional service and education regarding kidney health and treatment options
• Engage members in HealthMap's Kidney Health Program and follow up based on policy guidelines
• Coordinate care among patients, their support networks, and healthcare providers to resolve barriers and assist in meeting individual health goals

Required qualifications

• Active, unrestricted RN license required
• Bachelor's degree required; or 6+ years of RN experience including 3+ years in case management may be considered in lieu of degree
• CCM certification preferred
• Three (3) years of experience in case management preferred
• Experience in a dialysis center or transplant center preferred
