Job Summary
To support patient navigation in the healthcare system, the full-time Remote Transitions of Care RN Case Manager will provide telephonic, short-term interdisciplinary care management for high-risk patients post-discharge, ensuring they receive necessary resources and support for improved health outcomes.
Key responsibilities
- Engage with newly discharged patients to assess needs and develop personalized care plans
- Coordinate with healthcare providers to ensure comprehensive follow-up and seamless care transitions
- Educate patients on chronic conditions and facilitate access to community resources for ongoing support
Required qualifications
- Unrestricted Compact RN license in the United States with the ability to obtain other state licenses
- 3-4 years of RN care management or telephonic case management experience
- Availability to work Monday-Friday from 8:30 AM to 5:00 PM MST/PST
