RN Case Manager - Remote

Job Summary

Seeking a full-time RN Case Manager - Remote, who will manage care for members with complex and chronic needs by assessing, developing, implementing, and evaluating care plans while working primarily through telephonic communication.

Key responsibilities

• Ensures members have access to appropriate health services based on their needs
• Identifies and manages members according to established criteria, facilitating necessary authorizations and referrals
• Monitors and evaluates the effectiveness of care management plans, making modifications as needed

Required qualifications

• Minimum 5 years of clinical case management experience or equivalent education and experience
• Associates or Bachelor's degree in Nursing; BSN preferred
• Current, unrestricted RN license in the applicable state
• Ability to communicate effectively and collaborate with diverse groups
• Strong problem-solving, organizational, and time management skills
