RN Clinical Eligibility Nurse

Job Summary

To support a growing virtual cardiac rehabilitation program, the fully remote Clinical Eligibility & Intake Nurse will review EMR records for medical eligibility and conduct initial patient assessments, working up to 40 hours per week, Monday through Friday, between 8 am and 10 pm ET.

Key responsibilities

• Reviewing patient charts in EMRs to determine medical eligibility for the cardiac rehab program and documenting decisions for the enrollment team
• Conducting initial assessments via video for new patients, including reviewing medical histories and creating individualized treatment plans
• Communicating effectively with patients about program benefits and engaging them through motivational interviewing and goal setting

Required qualifications

• Active RN or LPN license; multistate/compact license preferred
• 3-5 years of clinical experience in cardiac care, cardiac rehab, or chronic disease management
• 3 years of experience with EMRs, particularly Epic, including chart review
• Working knowledge of cardiac conditions, procedures, and medications
• Experience assessing and educating patients, preferably older adults, in a clinical or virtual care setting
