Job Summary
Accountable for financial and clinical performance in a full-risk environment, the remote RN Director of Care Management will lead the development and management of comprehensive care management programs, focusing on reducing avoidable utilization and improving outcomes for high-risk patients while working within the Central or Eastern time zones.
Key responsibilities
- Provide strategic leadership and oversight for care management programs and services

Required qualifications
- Masters level degree (MSN, MBA, or MHA)
- Bachelors level degree in nursing (BSN)
- 8+ years of progressive leadership experience in care management or value-based care
- 3+ years' experience in full-risk population management
- Significant experience in change management and operational excellence
