RN Head of Care Management
Job Summary
As the clinical lead for Clara's care management efforts, the full-time RN Head of Care Management will ensure adherence to clinical protocols, oversee quality across programs, and make critical operational decisions while working remotely.
Key responsibilities
- Translate clinical protocols into Clara's conversation design and define triage logic for patient escalations
- Build and manage Clara's clinical quality program, tracking metrics and resolving clinical incidents
- Serve as the final clinical sign-off for new workflows and partner with customers during implementation and reviews
Required qualifications
- Active, unrestricted RN license
- 7+ years of clinical nursing experience, including 5+ years in care management
- Hands-on experience with transitional care and chronic care management programs
- Working knowledge of quality programs (HEDIS, Stars) and CMS care management requirements
- Experience in building or improving care management protocols and workflows
