RN Senior Manager, Care Management

Job Summary

Seeking a transformational clinical leader, the full-time RN Senior Manager, Care Management will manage a team of 60-70 frontline RNs to deliver personalized care during the critical 30-day post-discharge period, ensuring safe transitions for members in a remote setting.

Key responsibilities

• Cultivate and inspire a high-performing team focused on improving post-acute patient experiences and outcomes
• Drive hiring, training, and performance management strategies for frontline teams and managers
• Track staff performance against productivity and quality KPIs to reduce avoidable admissions and readmissions

Required qualifications

• 5+ years of experience leading clinical teams in transitional care or care management
• 3+ years of experience in coaching and developing other leaders/managers
• Active RN license in good standing, with a BSN or equivalent degree; master's preferred
• Experience in post-acute medicine, nursing, and case management processes
• Experience managing interdisciplinary care teams with a strong conviction for collaborative care
