Seasonal Nurse Practitioner/Physician Assistant

Job Summary

To support a mission-driven clinical team, the full-time Seasonal Nurse Practitioner/Physician Assistant will conduct telehealth visits, perform comprehensive assessments, and deliver preventative care from a remote location during the peak seasonal period from November through March 31st.

Key responsibilities

• Conduct telehealth video visits and occasional in-home assessments for members
• Provide evidence-based care focused on prevention and chronic condition management
• Collaborate with care team members to create individualized, whole-person care plans

Required qualifications

• Active Nurse Practitioner (APRN/NP) or Physician Assistant (PA) license
• 3+ years of outpatient clinical practice experience, preferably in primary care or geriatrics
• Active BLS certification at the time of hire
• Comfort with delivering care via video telehealth and using electronic documentation systems
• Ability to commit to a full-time schedule for the seasonal assignment
