SNP Care Coordination Supervisor

Job Summary

Overseeing a team of Care Coordinators, the full-time remote SNP Care Coordination Supervisor will manage care coordination for members with complex needs, ensuring compliance with program requirements and fostering team engagement.

Key responsibilities

• Monitor and support timely completion of Health Risk Assessments and Individualized Care Plans for SNP members
• Develop and oversee SNP Care Coordination programs and workflows, including standard operating procedures and performance reports
• Mentor and manage a team of non-clinical Care Coordinators, ensuring adherence to policies and regulatory requirements

Required qualifications

• 3+ years of experience in Care Coordination or equivalent education and experience
• 1+ years of supervisory or leadership experience in a healthcare setting
• High school diploma or GED; relevant experience or training preferred
• Knowledge of Managed Care Plans and medical terminology
• Ability to analyze data for trends and communicate effectively in English
