Job Summary
Supporting New York's Medicaid Section 1115 Waiver program, the remote hourly Social Care Coordinator will manage member communications, schedule non-clinical assessments, and coordinate referrals to medically tailored nutrition services and other health-related social needs.
Key Responsibilities
- Complete training for total access to Social Care Network (SCN) platforms
- Schedule and support completion of non-clinical assessments while ensuring timely follow-up
- Coordinate referrals across internal teams and external community-based organizations (CBOs)
Required Qualifications
- 3-5 years of customer service or member engagement experience
- Knowledge of Medicaid and/or Health-Related Social Needs (HRSN) programs
- Proficiency in CRM or care management platforms
- Strong communication skills
- Problem-solving and active listening skills
