Social Service Specialist
Job Summary
Working remotely, the full-time Social Service Specialist will engage with clients by phone to assess needs related to Social Determinants of Health (SDOH), provide information on benefits eligibility, and facilitate access to community resources.
Key responsibilities
- Respond to calls from clients' members or patients seeking assistance with eligibility for benefits and access to social services
- Conduct standardized assessments to identify members' needs and eligibility for available resources
- Document all member interactions accurately and maintain records in the designated database systems
Required qualifications
- High School Diploma or GED equivalent
- At least 1 year of experience in a professional office and customer service center environment
- 1+ year of experience in social services or healthcare preferred
- Proficient in MS Office and Windows operating systems
- Basic knowledge of social services and challenges faced by underserved populations
