South Carolina Licensed LPN Care Coordinator

Job Summary

Supporting multiple high-volume outpatient gastroenterology clinics, the full-time South Carolina Licensed LPN Care Coordinator will manage Epic In-Basket tasks, triage GI symptom calls, and coordinate patient follow-ups to ensure high-quality care.

Key responsibilities

• Triage GI symptom calls and respond to patient inquiries regarding medications and advice
• Process medication refill requests and coordinate prior authorizations for medications
• Assist RNs with care coordination and streamline transitions of care after testing and hospitalizations

Required qualifications

• Current South Carolina LPN License or compact state license
• Graduate from an accredited Practical Nurse program with at least six months of patient care experience
• Current American Heart Association (AHA) Basic Life Support (BLS) certification or American Red Cross BLS for Healthcare Providers certification
• Ability to demonstrate commitment to patient-centered care and effective communication skills
• Computer literacy and proficiency in managing electronic health records
