Job Summary
To support a thriving primary care team, the full-time South Carolina Licensed Master Social Worker will assist patients and families in making psychosocial adaptations, facilitate eligibility for social programs, and collaborate with an interdisciplinary team, primarily in a remote setting with occasional onsite meetings in the greater Charleston area.
Key responsibilities
- Assist patients and families in adapting to psychosocial challenges related to their diagnoses and access social programs and financial resources
- Utilize health care services and community resources effectively, aligning with human growth and development principles
- Assess, plan, implement, and evaluate individualized care plans in collaboration with the interdisciplinary team to achieve optimal patient outcomes
Required qualifications
- Master's degree in social work or social welfare from an accredited program
- Licensed Master Social Worker (LMSW) licensure by the South Carolina Board of Social Work Examiners
- Certification in Case Management preferred
