Job Summary
To support an integrated care delivery program, the full-time South Carolina Licensed Social Worker will conduct comprehensive psychosocial assessments, develop person-centered care plans, and provide case management interventions remotely.
Key responsibilities:
- Conduct comprehensive psychosocial assessments of patients and families to identify strengths and needs
- Develop and implement person-centered care plan goals in collaboration with patients, families, and caregivers
- Facilitate communication among patients, families, and the interdisciplinary care team to support care goals
Required qualifications:
- Master's Degree (MSW) in Social Work
- Current unrestricted LCSW or LICSW license in the state of South Carolina
- 2+ years of social work experience (internship not included)
