South Carolina Licensed Transition Care Nurse

Job Summary

To support value-based care initiatives, the full-time South Carolina Licensed Transition Care Nurse will manage telephonic outreach to patients discharged from the hospital, ensuring timely follow-up care and thorough documentation while preventing unnecessary acute care utilization.

Key responsibilities

• Conduct telephonic outreach to patients post-discharge and facilitate timely referrals to community resources
• Document call summaries and reinforce discharge plans to ensure follow-up with primary care providers
• Assist in compiling data metrics to measure the impact of transitional care initiatives

Required qualifications

• Bachelor's degree in nursing required
• Licensure as a registered nurse by the South Carolina Board of Nursing or a compact state
• Two or more years of experience as a clinical RN
• Prior experience in community health education, case management, or primary care preferred
• Current American Heart Association (AHA) Basic Life Support (BLS) certification required
