South Carolina RN Assessment Coordinator

Job Summary

To support home health services, the remote South Carolina RN Assessment Coordinator will complete evaluations, assessments, and treatments while ensuring timely documentation and maintaining continuity of patient care.

Key responsibilities

• Complete evaluations, assessments, and treatments in accordance with policy and individualized care plans
• Ensure timely and accurate documentation and records
• Follow established policies and procedures related to the scope of practice

Required qualifications

• Current, active, and unrestricted Registered Nurse (RN) licensure in South Carolina
• Active ICD-10 Certification
