Job Summary
To support patient care initiatives, the full-time State Licensed Care Coordinator will manage a panel of 400-450 patients, providing health coaching and conducting outreach from a HIPAA-compliant home office environment.
Key responsibilities
- Conduct monthly outreach and daily calls to patients, delivering personalized care plan support and health coaching
- Collaborate with clinical teams and providers to address patient needs and ensure continuity of care
- Document all patient interactions in real time while maintaining HIPAA compliance and call quality standards
Required qualifications
- Must hold a nationally recognized medical assistant certification (CMA, RMA, CCMA, NCMA, CMAC, or equivalent state-issued MA-C) or be a Certified Pharmacy Technician (CPhT)
- Minimum 1 year of clinical experience in a primary care, hospital, or specialty healthcare setting
- Experience with Electronic Medical Records (EMRs) and strong computer navigation skills
- HIPAA-compliant home office setup with minimum 50 Mbps download / 5 Mbps upload internet capability
- Must be legally authorized to work in the United States
