Job Summary
To support a growing healthcare initiative, the full-time State Licensed Clinical Pharmacist will conduct telephonic medication consults, optimize medication therapy, and collaborate with healthcare providers while working remotely.
Key responsibilities
- Conduct medication consults with members, focusing on medication adherence and quality metrics
- Identify and address medication adherence barriers, collaborating with patients' providers and pharmacies
- Participate in team engagements and coaching sessions to enhance clinical interaction skills and performance
Required qualifications
- Active Pharmacist License in the state of residence
- Graduation from an accredited pharmacy program (PharmD) and passing the NAPLEX
- 1+ years of Clinical Pharmacy Practice experience
- Experience with Medication Therapy Management (MTM) and Comprehensive Medication Reviews
- Proficiency in Microsoft Word, Excel, Outlook, and PowerPoint
