State Licensed Clinical Pharmacist
Job Summary
To support a growing healthcare initiative, the full-time State Licensed Clinical Pharmacist will conduct medication consults, identify medication adherence barriers, and collaborate with healthcare providers in a remote setting.
Key responsibilities
- Conduct telephonic medication consults that meet quality metrics and address member-specific medication adherence barriers
- Implement a member-centric approach in clinical consults and support patients in therapy initiation and maintenance
- Participate in team engagements and coaching sessions to enhance clinical interaction skills and performance
Required qualifications
- Active State Pharmacist license in the state of residence
- Bachelor's Degree in Pharmacy or a related field; PharmD preferred
- 1+ years of Clinical Pharmacy Practice experience
- Experience with Medication Therapy Management (MTM) and Comprehensive Medication Reviews
- Call Center experience (1+ years) is desirable
