State Licensed Clinical Pharmacist
Job Summary
To support a Clinical Engagement Services team, the remote Clinical Pharmacist will conduct telephonic consultations, perform comprehensive medication reviews, and provide education on medications and disease states.
Key responsibilities
- Conduct telephonic consultations and comprehensive medication reviews
- Identify gaps in care, adherence concerns, and drug interactions
- Document clinical interventions and communicate recommendations to patients and healthcare providers
Required qualifications
- Current, unrestricted Pharmacist license in the state of residence
- Strong knowledge of Medication Therapy Management (MTM) and disease-state management
- 1+ year of remote/independent work experience
- PharmD and PBM/retail experience are preferred
- Bilingual skills are a plus
