State Licensed Clinical Pharmacist
Job Summary
To support a Clinical Engagement Services team, the part-time contract State Licensed Clinical Pharmacist will provide telephonic clinical consultations, conduct comprehensive medication reviews, and support medication adherence and disease-state management in a fully remote environment.
Key responsibilities
- Engage members telephonically and schedule consultation appointments for clinical programs
- Conduct comprehensive Medication Therapy Reviews and provide clinical recommendations
- Educate members on medications, regimens, and relevant disease states
Required qualifications
- Current and unrestricted pharmacist license in the state of residence
- Strong knowledge of disease-state management guidelines and MTM principles
- Approximately 1+ year of pharmacy experience preferred; new graduates may apply
- Ability to work independently and effectively in a cross-functional team environment
- Intermediate proficiency with Microsoft Word, Outlook, and Excel
