Job Summary
To support a growing healthcare initiative, the full-time temporary State Licensed Clinical Pharmacist will conduct clinical consults via telephone, optimize medication therapy, and collaborate with healthcare providers while working remotely from Monday to Friday, 9 AM to 5:30 PM EST.
Key responsibilities:
- Conduct medication consults and provide adherence counseling to members, ensuring quality metrics are met
- Identify and address medication adherence barriers in collaboration with patients' providers and pharmacies
- Participate in team initiatives and support Advanced Practice Clinicians through telephonic consultations
Required qualifications:
- Active Pharmacist License in the state of residence
- Bachelor's Degree in Pharmacy or a related field
- 1+ years of Clinical Pharmacy Practice experience
- Experience with Medication Therapy Management (MTM) and Comprehensive Medication Reviews
- Proficiency in Microsoft Office Suite (Word, Excel, Outlook, PowerPoint)
