State Licensed Health Coach
Job Summary
To improve health and wellness for patients with chronic conditions, the full-time State Licensed Health Coach will develop patient relationships, monitor adherence to care plans, and communicate findings to the cardiology care team in a remote setting.
Key responsibilities
- Develop a patient coaching relationship that supports care plans and enhances quality of life
- Monitor patient adherence to care plans and communicate abnormal findings to the care team
- Support new patients in the enrollment process and act as a liaison between patients and providers
Required qualifications
- High School diploma or GED
- 1 year of experience in remote chronic care management
- Experience with cardiology patients, specifically heart failure
- Registered Nurse (RN), Medical Assistant (MA), or Certified Nurse Assistant (CNA) certification
- Experience with EMRs, particularly Athena, NexGen, GEMMS, and e-clinical works
