State Licensed Health Coach
Job Summary
To improve health and wellness for patients with chronic conditions, the full-time remote State Licensed Health Coach will develop supportive patient relationships, monitor adherence to care plans, and communicate effectively with the cardiology care team.
Key responsibilities
- Develop and maintain patient coaching relationships that enhance care plans and quality of life
- Monitor patient adherence to care plans and communicate abnormal findings to the care team
- Assist new patients in the enrollment process and act as a liaison between patients and providers
Required qualifications
- High School diploma or GED
- 1 year of experience in remote chronic care management
- Experience with cardiology patients, particularly heart failure
- Registered Nurse (RN), Medical Assistant (MA), or Certified Nurse Assistant (CNA) certification
- Familiarity with EMRs, specifically Athena, NexGen, GEMMS, and e-clinical works
