State Licensed MTM Pharmacist

Job Summary

To support a growing clinical services team, the remote State Licensed MTM Pharmacist will conduct telephonic consultations, perform comprehensive medication reviews, and document care while adhering to HIPAA regulations in a contract-to-potential-FTE position.

Key responsibilities:

• Engage members telephonically and schedule pharmacist consultation appointments for clinical programs
• Utilize prescription claims data to generate clinical recommendations and assess medication regimens
• Document patient care notes and participate in departmental meetings to enhance clinical programs

Required qualifications:

• Current and unrestricted Pharmacist license in the state of residence
• Strong knowledge of disease state management guidelines and the 5 Core Elements of MTM Services
• 1+ years of experience working remotely/independently in a cross-functional team environment
• Intermediate proficiency with Microsoft Word, Outlook, and Excel
• Ability to work a flexible schedule with rotating shifts between 9:00am and 8:00pm CST, Monday through Friday
